The case manager (RN) is responsible for the clinical, quality, and cost effective outcomes for a caseload of patients. The case manager facilitates coordination of an interdisciplinary plan of care to guide patient progress. The case manager mobilizes resources and manages the system in response to patient needs according to an interdisciplinary plan of care. The position collects, trends, and analyzes aggregate outcomes data for that population to be used in identifying best practices and targeting performance improvement efforts. The case manager operates independently, working cooperatively and collaboratively with patient, families health care providers, payers, and management. Position requires self direction, motivation, organization, and interpretation of information relating to a wide variety of policies, procedures, and programs.
Job Requirements
Three years recent clinical experience in area of specially preferred. Must have current RN license. Current certification specially case manager field preferred. Must have excellent verbal and written communication skills. Ability to do multi task.
Education
Masters degree in nursing/healthcare related and administrative field preferred.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
